May 25, 2002 2:30 am, Saturday

for those of you who don't already know, Jess was hit by a car causing a leg fracture and severe blow to the lower left back of his head.  He was quickly transported to the trauma unit of San Francisco General by Tod who was first on the scene and instrumental in getting Jess the immediate care he needed, and getting him quickly to the trauma unit via ambulance.  Charles, a social worker at SF General, immediately contacted us, greeted us at the emergency room, and provided comfort and information.

At the trauma unit, Jess was immediately taken into surgery by Dr. Larson, a neurosurgeon, who removed part of Jess's skull, a normal procedure to release the pressure building on his brain.  He was then transferred to I.C.U. where he is now in critical but somewhat stable condition.  He is in a non-induced coma, on life support, with a private nurse at his side 24-hours a day.  Julio is his first nurse.

Inspector Jim Ramsey from the night investigations unit of the San Francisco Police Department, and members of his staff, were on the case immediately (3 am), and were able to locate the hit-and-run car. Jim, along with Inspector Hall, and officer John Mulkern are all following up on the event.  We are thankful for their professional and thorough efforts.

Jess was visited by many of his close friends and family.  Thank you for all of your support.  Your presence, and physical and emotional touches were a blessing and we are confident they were all felt in some way by Jess.

May 25, 2002 7pm, Saturday
the staff of the I.C.U. unit advised us that due to potential infection and other complications, only Heather and Jess's immediate family are permitted to visit him from this time until he is transferred out of I.C.U.

Doctors advised that the next three days are very critical in his recovery.  We understand that during this period of time the brain will respond to the trauma by swelling and will then hopefully begin returning toward normal.  As he begins to come out of his coma, they will know more about the extent of his injuries.  We will post this information daily.

We gathered from a phone conversation with his nurse, Jess was expressing some eye movements, coughing, and starting to lift his left arm which we understand to be the first very early signs they look for in the process of coming out of his coma.

May 26, 2002 6:45 am, Sunday
Diane was Jess's nurse through the night, no negative turns...
Jess had a good and uneventful night.  His vital signs are still somewhat stable.  His level of response has not improved from yesterday.  A neck down X-Ray yesterday revealed only the leg fracture, but no apparent other internal injuries.  He is passing and receiving significant levels of fluids which does not appear to be problematic. 

Cranial (brain) pressure has remained at reasonable levels throughout yesterday and throughout the night.  This is the goal everybody is now working toward to maintain quality of life for Jess and pave the way toward reasonable recovery.

We appreciate all your prayers and thoughts ... Personally, in the short term, We are trying to visualize keeping Jess's brain pressure low ...

May 26, 2002, Sunday morning visit ...
Rivka, a nurse at the hospital spent time with us last night sharing her 18 years of experience with this type of trauma.  She helped us get a grip, trust the medical staff, and get a good night sleep in preparation for times ahead.  She said "let us handle the numbers ...".  This allowed us to arrive refreshed this morning and get a look at Julio, Jess's profesional nurse, artfully performing his magic of Life Support.

May 26, 2002 10pm, Sunday
Great presence of friends again at the hospital today.  Doctor expressed cautious optimism.  Word was Jess's pupils might be expressing a positive trend.  Still not the voluntary response signs we want.  And a disturbing hint about how things may not be quite the same...

We are in the middle of the 72 hour critical period where Jess's brain is expanding on its way toward hopefully contracting.  Seems to us this evening through tomorrow is a critical period. Please help us visualize keeping the brain pressure low...  we have a ways to go ...

We just talked with Diane, his night nurse, and his brain pressure has gone up slightly, and she has had to drain the fluid twice in the last two hours to keep the pressure at an acceptable level.  If it gets to high, they may have to do another operation to remove more skull bone to allow for expansion, but this is only after other options have been exercised.

Visualize the brain pressure low ... allow for the expansion ...

Diane called and said Jess had a reasonably stable night ...
He experience one setback during the night,
a collapsed lower lung,
which they were able to stabilize ...

Jess has such an amazing network of friends,

we are amazed and grateful ...

The day started with a report from Diane that Jess had a stable night, but experienced a collapsed lung (lower area)  in the night and was developing a little pneumonia in the lower part of one of his lungs.  Upon arrival at the hospital Julio pointed out to us that the sedentary nature of a person on life support sometimes leads to an infection in the lower part of the lungs due to a lighter than normal circulation of air in that area.  He said they were removing the resperator periodically and blowing (bagging) air deep into his lungs to help the situation.  All seemed well with Jess by the time we arrived.  He was also developing a bit of a sinus infection.

The day unfolded well.  Jess had made it through another critical period, and Julio was kind enough to allow visitor after visitor as he took over the orchestration of life support from Diane.  Quite simply put, they are pros.

We all felt a hint of Jess's unfolding presence and talked about how he has always been very cautious about how he approaches situations.  We concluded that he would appear when he wanted to appear, and would probably be a bit expressive upon arrival.  We talked about future BBQs.

Last night, Sam and Mollie brought Jess's 1978 Chevy Truck to our house for temporary storage.  As we were moving it this morning, we took a picture of it and captured the sound of it starting up as we moved it to another location in the driveway.  We put the picture and sound up on this webstie (see "Jess, here's your truck" link above).  We took our laptop computer into his room and played it for him.  His new dual exhaust mufflers, installed just weeks earlier, rumbled in the room.  Suddenly his shoulders moved and eyes fluttered to a larger extent that we had seen previous.  Before long, we realized that the short starter motor and rumbling audio sequence was playing over and over.  It seemed he was suddenly aware and concerned about what we were doing to his truck.  Needless to say, we were elated... Lot of fun, alot of laughs ...

Anyway, the consensus is that Jess's trend is positive.  His pupils are still not reacting satisfactorily to light, but his aura is quite apparent.  All the numbers seem stable and reasonable.  No persistent negative turns. We feel a turning point ...

He is scheduled for an MRI tomorrow (Tuesday) morning.  After the MRI is evaluated, we will be talking with his primary doctor and will let you know.  He is also stable enough to be scheduled for surgery to set the bone fracture in his lower right leg on Wednesday.  This was put off due to the priority nature of his head injury.

Jess's youth, and the professional environment he is in are his saving graces.  We were told quite frankly that the situation is serious, and for an older person would have most likely been fatal.

Can't express how thankful and lucky we are,
and we are preparing ourselves for the long haul.

Kyle is OK with it,
Deborah is OK with it,
and Rik is OK with it.

Leo has not been to the hospital yet to see Jess.
But we might take in a virtual "Bark"

A little optimistic?  Yes!
And are we fully aware that things could change at any moment?  Yes!

The online journal is really growing.

May 28th, 2002 6 am Tuesday

Just got a call from Tina his new night nurse and Jess had a stable night with some great news - there is some reaction to light in his pupils!!!!  She emphasized "slight" but enough reaction that she noticed it!!!!!!!!!!! 

This is the sign we (and they) have been waiting for ...

They are pushing more fluids as he is "dry" and is scheduled for MRI sometime today. 

We read the journal entries from you to Jess each day.

We were visited today by Marco Garcia,
and his brother Marvin.

Marvin was told back in January that his brother Marco was not going to make it.  The odds they gave were 99 to 1 he would shortly no longer be with us ...

Marvin's religious beliefs and some other significant conversations proved that the death of his brother was simply not an option.

Marco, four months later, is alive, well, and was a source of inspiration to us all today.

Jess's doctor today said that discussions are not longer about death,
and we all felt Jess's presence today,
and we are confident he wants to be with us.
It is now  more about the physical process and signs of recovery...
The lights to the house are coming on,
the vital signs are trending supportive,
Jess now needs to show us voluntary reaction.
and they say he needs to do it sooner rather than later,
we have a new mission ...

Nick and Colburn were calling him out today,
and the medical staff was supportive ...

Within a few days some roads will begin to diverge in the woods,
and we want to be on the right one.

Please keep in mind as you read the detailed report following from Kristin Jadrnicek, that nothing was said by the doctor today about death, actual irreversable destruction, or other actual negative happenings.  It was all about needing evidence of the process of recovery.  

Kristin Jadrnicek's report

Spoke to Dr. Quinones who is the chief resident surgeon at the hospital. He said... The staff is looking at Jess on a minute by minute basis. Things are changing daily and it is impossible to predict what will happen next. What is true for today may not be true for tomorrow.  Jess has many things going for him...
His age  The fact that he has survived past the crucial 72 hour
mark. 

All his vital organs are fine and unharmed from the accident.  Jess can now breath without a ventilator (He is still on one to keep lots of oxygen going to his brain to help his brain heal).  The pressure in his brain is now, and has continued to
stay, at good, low levels.  His pupils have slight and sluggish responsiveness to
light.  His eye lids are fluttering, making the Nurse Rivka hopeful that in another 48 hours or so he may open his eyes.  He has movement in his left arm.

The Doctors are working very aggressivly to make sure that Jess's body is fully supported to give him every possible chance to recover.

Jess has some things going against him...
Given the low pressure levels in his brain the Docs feel that he should have a certain level of motor responsivness that thus far he is not showing.
At this point he should be waking up and that at this
moment has not happened.  He is non responsive to pain stimulis.
HOWEVER, THESE THINGS ARE NOT EXACT INDICATORS!

Now... here comes all the explanations
One of the things that the Docs use as a universal measure is something called a Glasko Coma Scale.  This test is administered at the time of admission to
the hospital and has a range of 3-15.

At the level of three (3) the patient is non verbal, has no eye response, and has no moter response, it is the lowest on the scale. The level of fifteen (15) is a normal, healthy person. This gives the Docs a universal measure to estimate how  tramatized the head is. At the time of Jess's admission to the hospital, his number was 4.  He had no verbal, no eye movement and only a little motor response.

They also found that, upon arrival, he had Hypoxia, that is the level of oxygen in his blood was very low. Given that blood is needed to carry glucose and oxygen to feed the brain, Jess's brain was limited to receiving oxygen for an undetermined period of time. This could have created what is called a secondary brain injury due to the brains lack of oxygen. This they do not know for sure, but consider it a possibility due to the Hypoxia.  Typically a brain cannot go more than 6 min. without oxygen.

Another concern for the Docs is a condition called Axonial injury. Until they can perform an MRI they will not be able to tell if Jess's brain has sustained
this injury.  Axonial injury is the tearing of the connective tissue between the Grey matter of the brain (The area of the brain that handles the more complex functions and the White matter of the brain (The area that handles the more automatic and base functions of the brain)(A bit sketchy on that explanation) If this tear has occured between the Grey and the White matter than there is no treatment that is available. I will get a clearer picture tomorrow as to what the ramifications of this
are medically.  These are possibilities that they are concerned with
because of Jess's lack of responsivness thus far.
AGAIN THESE THINGS ARE NOT EXACT INDICATORS!!!!!!

As for those of you wondering about his leg injury he recieved a broken Tibia and Fibula bone. The break occured in the middle of his lower leg. There is a clean break and a deep fracture in the Tibia bone with only what looks to be a break in the Fibula. The Docs are not going to do anything to his leg at the moment. They do not want to create a condition for Jess that could drop his blood pressure and possible create a secondary brain injury (An injury due to lack of
oxygen remember) The Docs want to leave the leg until they are confident that laying his head down, or putting his body under a lot of stress (Like a bond
surgery) will not injure him further.  Same with laying flat for 1-1/2 hours for an MRI.

The final outcome of the meeting was that the Doc's said that the odds were not in his favor. They said that the longer he takes to wake up and the longer he
has no signs of life the greater the odds he needs to overcome on the way to having a meaningful recovery.

They did emphasize that they are working on limited information at this time, that they cannot give any predictions good or bad and that anything could happen. They said to NOT GIVE UP HOPE!

What they need to see from Jess is more complex movements in his body. They need him to become responsive to the pain that they give him as a tester.
He needs to open his eyes and start to come to.

What is going on is Jess's brain is reconnecting and the Doc's just need to see the signs of response from him and his body. A few more things tomorrow as I want to ask Rivka a few questions for clarity purposes.

Not good ...
Arrived at the hospital and Jess was being taken for a CAT scan because his cranial pressure and blood pressure had spiked, and they wanted to know why ...

Upon return the team of doctors and nurses wanted to have a family conference.  Basically the CAT scan  revealed that 3/4 of his brain appears to be  non functional ...  and it appears that there is nothing they can do about it.

The part effected is the dominant part of the brain.  Kristin will provide details in awhile.

The bottom line is they feel they can keep him on life support for a few days with the possible aggravation of continued brain swelling, at which time they may have a difficult time controlling the situation.

Short of a miracle, there appears to be a fork in the road of recovery.

We will be heading up to Bolinas Ridge with friends to see if we can get some hint of Jess's intentions ...

Kristin Jadrnicek's Update:

Hello everyone, here is the update....
We spoke with the Docs today around 2:00 today and this is what they had to say,
Early today Jess's head pressure and blood pressure went up drastically. Nurse Rifkin drained his head several times to no avail. Also Jess's pupils had become unresponsive again. He was not moving his left arm nor was he responding to the basic motor tests that they give him. They decided to take Jess down to do a CAT scan and see what the situation was with his brain. The scans reveled a number of things,
First, that the Hypoxia (lack of oxygen to the brain) that created a possible secondary brain injury has happened. Apparently Jess's brain went without oxygen for a substantial  amount of time before he reached the hospital and this did create an additional injury to his brain. Also the scans reveled that both Occipital lobes are infarcted and the Basil Ganglia is also infarcted although he does have a bit of brain stem activity. On the left side of his brain there is no difference between the Grey matter and the White matter (See last report for explanation) and for all us non Medical types I will attempt an explanation.........here it goes.
Basically 3/4 of his brain is not able to function as a result of the lack of oxygen and the trauma sustained in the impact. Both the higher and lower functions of this part of his brain are now unable to work. The Occipital lobes basically being the higher functions and the Basil Ganglia is at the top part of his brain stem. (The brain stem is  what controls the involuntary actions of the body such as his eye movement and his breathing.)

 Basically it is as if Jess had a major stroke and it permanently injured the left side and a quarter of the right side of his brain. The last 1/4 of his brain is functioning alright. The Docs were not optimistic about his prognosis or his hope for a recovery. They feel that he will not wake up.
From here we asked what was the next step for Jess, they said that they could control his brain swelling for awhile but that there would come a time when their efforts would become ineffective and that the injured parts of Jess's brain would die. The Docs feel at this point that there is nothing that they can do to prevent this outcome.
I must say that all the Docs and Nurse Rivka were surprised, and disappointed at this outcome.
PLEASE DO NOT GIVE UP HOPE Although this looks discouraging he still has a functioning quarter of his brain and there are still posibilities despite the way things look

KEEP UP THE GREAT PRAYERS AND ENERGY AS WE ARE ALL CONFIDENT THAT JESS IS GETTING THEM

What we need to do now is support Jess in the decision that he makes about whether or not he wants to stay with us or move onward.

The sun is rising on a new day with Jess ...
Called this morning and not much change from yesterday ...
He is stable but cranial pressure and blood pressure still high ...

We will try to get clearer pictures and possible animation up as time permits.

We can remember when Jess fell and got a little cut under his chin (see pic).  He got three stiches.  He was climbing up the bleacher steps at Tam High and went straight for the ONLY little hole in the bleachers and fell 2 - 3 feet on his chin.  We were devastated, and felt like the worst parents in the world ...

Among his friends on Bolinas Ridge last night,
we became so aware of his strength,
He has affected so many lives,
and  comes full circle on so many things in his life,
only one of which is showing his love for us as parents ...

Arriving at the hospital this morning we found Jess in such good care with his nurse Liz.  She made it so easy for us to arrange a variety of visitors in the midst of administering her life support magic ...

Following Ryan days earlier, Brian, a neurosurgeon resident, appeared as he always has at germinal moments since the beginning of Jess's ordeal.  We suddenly realized, and appreciated, that he, Bob (head of the I.C.U.), Kristine (clinical specialist), and Stephanie (social worker) would appear at just the moments we needed help.  Stephanie would step into our private moments and plant seeds of comfort.  These professionals are so good as vehicles of information and comfort that they appear to go unnoticed until you realize what they have done.  We are sure that regaining our consciousness will uncover more people that we have not yet acknowledged for their support. What a team ...

Cranial pressure still under medical control, internal injuries still keeping the pressure forces temporarily on hold and waiting to continue when given a chance that Liz will never allow ...

Karl and Elise Jadrrnicek revisited Jess and found his life force moving in and up into his chest and head area.  They felt a movement also toward the undamaged part of his brain ...

Luke and Gregor arranged a mid morning visit from the Tibetan Rimpoche living in Mill Valley, California.  He knew Jess and shared barbecues with him.  They performed a ceremony for Jess and told us they will be praying for him and would assist him in his journey.  It was very comforting to have him there.  He confirmed the life force moving into the upper part of Jess's body.

They told us to call him when Jess passes and to stay with his body for half a day and he would assist Jess on his journey.

Jess's friends:  What more can I say, their strength and love continues to grow minute by minute, hour by hour, day by day, 24/7/365 and no doubt, forever ...

Kristin Jadrnicek's Update.

Hi everyone,

The news is short and simple this evening. Since the news about Jess's brain on Wed. there has only been one change. Jess's ICP (cranial pressure) has stabilized to some degree and may not be completely dependent on the Manital (Salt to decrease brain swelling) to keep his ICP down in the future. However, Dr Manley stated that there is no change nor will there be any change for Jess's brain to have any functionality. It is damaged beyond repair or recovery. Given this information the family had a private meeting to discuss the options that were given to us all and what we all think Jess wants for himself . The outcome of that meeting was the peaceful decision to discontinue the ICP medication and to allow Jess's brain to do what it was going to do, either survive working at minimal brain function or to herniated and die due to uncontrollable brain pressure. The decision of what will happen when the Manital is discontinued is now up to Jess. It is the families decision to let nature take its course and be there when Jess decides stay with us or pass onto his next journey. We all believe that Jess would have wanted to donate his organs so when his body functions cease his organs will be donated. Thank you all..........................

Jennifer, his nurse was talking to us about getting Jess's dog Leo in for a visit.  Deb was saying Leo likes to sleep with Jess, so brining him in would require Leo getting up into the bed with Jess.  She seemed to think that might even be possible.  Can you believe the support we are getting from this medical team?

Jess is heading toward being put on his own physical resources as early as tomorrow afternoon, and as late as Monday afternoon...

Friends are visiting, and talking to him, and touching him, and being with him ... contstantly.  Jennifer is allowing this, Lori took over from Jennifer at 7pm for the night.  Thank you, thank you, thank you for giving Jess every chance to experience the tremendous love that surrounds him right in the middle of a busy I.C.U.

but last night they felt Jess feeling our pain ...
They felt the presence of the Tibetan monks that visited him,
Who are with him and ready to help him in any journey he decides to take.

Mascara streaming down some cheeks,

Mom & Heather on beck & call,
While Papa san’s computer tells it all.

 Is in & out of body Jessie’s plan?
Body’s warm, then cold, now minds outa hand!
Oh, how this brave young warrior struggles,
His friends providing love’s endless funnel.

 Where do we start, when will it end,
This eternal saga in life’s drama pen?
Why must it happen to such a good friend,
or is this answered when experiencing Oneness again.

I was looking for pictures of Jess the other night and came
across a negative of him and some other Team kids on the
top of Half Dome. I am leaving in a few minutes go there
(we will make the climb on Friday). He will be in my heart
and mind, just like he's there for real, when I go up.

Nancy helped Jess through the day yesterday.
not much change ...

Playing football in the hospital corridors was not received well by hospital staff.  Please show them the same respect for their space that they are showing for ours.

Friend after friend passed by his bed,
and spent time with Jess ...
Bob Marley music and posters at his side,

Lori helped him through the evening,
not much change ...

Off to see Jess,
and another meeting with the doctors,
His medical team has been so helpful and professional ...

This is Jess's mom and somehow I need to express how you all have given us the strength and courage to go on ......  Kristen left yesterday and I miss you so very much - you were such a calming force in all this chaos and were able to make things happen - my sister had to go home to Boston to her girls and my Mom leaves tomorrow.  There are no words..... you have given us a gift in your presence...  Life goes on and I know you all need to attend to your lives.  Kevin carried a picture of Jess into the sweat lodge yesterday - Mathew ( you will always be Pib to me) graduated and we stopped by to spend a moment with you all and we are so very proud of you and what you have done.  To have you all spend time on the mountain with us and hear all the stories and laugh and cry and be together in the space Jess loved so much is the greatest gift to us....... We love you all so very much - please be safe in your journeys home and hold us in your hearts.

Last night an Eagle Feather and Tribal Blanket
 were presented to our family
in honor of Jess.

The Eagle Feather is from the Big Horn Mountains,
located in south central Montana and northern Wyoming. 
 

The feather has flown over the Big Horn Mountains,
Big Horn Lake, and Big Horn River, Montana,
before it was given to Al. 


Matthew danced with these feathers on the Crow Indian Reservation at the Round House as a part of his dance outfit.  He also danced at the Oil Celebration in Poplar, Montana, and numerous other locations in the northern plains of the US.

 

Matthew knows the sacred meaning of the eagle feather within his Native American Heritage and respects it. 
He wanted his brother Jess to have a part of his tradition
 that Matthew holds so special and close to his heart.

 

Deborah slept like a baby in the blanket
for the first time since Jess's accident ...

 

The Eagle Feather was passed over Jess
prior to his cremation ...

 

We are planning an Indian Ceremony
one year from Jess's death ...

 

We felt so honored ...

Debbie and Ryan Collins,
brought over a special crystal for Jess ...

notice the "crystal carrier" canvas on the ground,
it took 5 or 6 of us to carry the crystal to it's resting place ...

 

Thanks for the inspiration of this wheel of life Chris ...
I told Jess a story one time about hiking in Tibet,
studying the Mandala concept ...

I was sharing some rice wine with locals at the tree line,
No words were said,
they would slowly flick their beads,
slightly moving lips indicating an inward mantra,
as they visually traveled their own wheel of life ...
a calmness would come over them,
as they integrated my presence ...
then we would all laugh ...
I learned so much ...


I have seen this same calmness come over Jess,
and have experienced the same hearty laughter,
and I know it will continue,

forever ...

We went up to Bolinas Ridge to watch the sunset.
We had Jess's ashes with us ...

Jess, I heard you blowing up the valley through the grass before the sun set, as you caressed your parents, giving thanks and honor. There was a calm to your voice while you told them how much you loved them and thanked them for a joyful life And then with the sun about to hit the water, you gathered yourself. For the next hour we got to see you as you have always been and the being you will always be, an angel of light. You filled the sky with your presence with a seldom seen brilliance. Your face in the clouds, your body as yellow blue green light stretching through the clouds to other worlds. The sun long set, you rose up to the oncoming night sky, refusing to cower, refusing to step down. You made the sky a sunrise in a higher horizon. The pulse of your light danced in the smallest of movements, colors shifting until yellows gave way to orange, blues to purple. As you left you sent a deer to your mom and bob's "natural mystic" to your father. Warm winds kissed your parents, telling them you were fine, that the wild mountain time was still alive in your sweet loving heart. And I stood proud to have you as family, and to be taught about how courage and strength that comes with generosity of spirit and love. Thank you dear Jesse, I will look for you often.

Joseph came by
to tell us about Jess's Crystal,
brought by Debbie and Ryan Collins.
He said the Crystal,
big as a large computer monitor,
was many thousands of years old,
Origins in Story, Arkansas
Ron Coleman Ranch,
and that it was perfectly placed in the yard
as a protection piece ...
He said the sunlight, moonlight, and all of our touches
will be good company for it.
We feel Jess will help us care for it,
and it will amplify his spirit ...

A raven landed in our cypress tree,
about 20 feet above,
and squaked at me for over an hour ...
Wish I was tuned in enough to know what it was squaking about ...

Attended arraignment of John Wong,
New charges being made and all now pending further investigation
now that Jess has passed away ...
Don't want to dwell on this,
but will post status occasionally ...
Just rest assured we are aware of, and on top of,
every step being taken
in the legal process ...
